TRT with Anastrozole

Overview

When testosterone is administered, some of it converts to estradiol via aromatase. For most men, this is not a problem — and some estrogen is beneficial. However, in men who convert at high rates and develop symptomatic high estrogen, an aromatase inhibitor like anastrozole may be clinically indicated.

General Framework

1. Confirm Elevated Estradiol: Anastrozole use should be based on confirmed elevated estradiol with associated symptoms — not prescribed routinely.
2. Anastrozole Dosing: Typically very low doses (0.25–0.5mg 1–2x per week) to bring estradiol into range without crashing it.
3. Labs Monitoring: Estradiol is monitored closely to ensure levels don't drop too low, which has its own negative effects.
